We are very keen to include children (defined as aged under 16) in this survey, but legally they can only take part with the permission of an appropriate adult who has responsibility for them such as a parent, grandparent, guardian, or teacher.
If you can ask a child or children aged 8 -15 to take part and give your permission for them to do so, please click on the button below. You’ll be asked to confirm your age and relationship to the child, then they can take over and complete their survey.
